On 11 October 2021, the Real Property Amendment (Certificates of Title) Act 2021 commenced, which abolished the Certificates of Title (CTs) and the control of the right to deal (CoRD) framework. All existing CTs have been cancelled and CTs will no longer be issued. Existing CTs will not need to be produced, and CoRD holder consent will not be required, for a dealing or plan to be registered. All existing Guidelines subject to this change are currently being reviewed and will be updated to reflect these changes. Plan form 6

This form will always be sheet 1 of the administration sheets. It is the responsibility of the surveyor to complete all panels on this form except the Subdivision Certificate and the Office Use Only panels for the deposited plan number and registration details. Inapplicable panels must be struck through.

Statements of intention

If it is intended to dedicate of roads or reserves appropriate statements should be provided in this panel.

Crown land NSW/ Western Lands Office approval

This panel will not be required for any community plans and should be struck through.

Subdivision Certificate

This certificate is completed and executed by the Authorised Person/ General Manager from the local council on behalf of the council or a Registered Certifier.

All appropriate information should be entered and the inapplicable information should be struck through.

Deposited plan number, registration details and plan purpose

These panels are for Office Use Only and must be left blank on plans lodged for registration.

NOTE: If the plan has a pre-allocated plan number or was pre-examined, the deposited plan number may be completed in font style Arial, Regular with a font size of 56.

Plan heading

The plan heading should be completed and indicate Plan of Subdivision of Lot___ in DP_____. The first plan in a scheme will always be a subdivision of a lot(s) in a deposited plan(s).

Local Government Area, Locality, Parish and County

The current Local Government Area (LGA), Locality, Parish and County should be shown in the appropriate position.

Survey Certificate

This panel must be completed and executed by the surveyor.

All appropriate information should be entered and the inapplicable information must be struck through.

Plans used

This panel should list all of the plans used in preparation of the survey.

Surveyors reference

Enter the surveyors reference as it appears on the plan. The reference must be identical to that shown on the plan to provide a link between the documents. Following the surveyors reference information in brackets may be added indicating version numbers of the document. This may vary between the plan and the Administration Sheets.
