(E) The applicant must be the Environmental Protection Authority or an appropriate regulatory authority as defined by section 6 of the Protection of the Environment Operations Act 1997.
(F) A brief description of the nature of the request must be provided, e.g. 'Request to record a Compliance Cost Notice pursuant to s106 Protection of the Environment Operations Act 1997'"
(G) Full details of the request must be provided, e.g. 'The applicant requests the Registrar General to record on the abovementioned folio of the Register that the land in that folio is subject to a Compliance Cost Notice pursuant to section 106 of the Protection of the Environment Operations Act 1997 issued by the applicant on 1 June 2008.'
(H) The dealing must be executed by the an authorised officer of the applicant, whose full name and position must be stated. The signature must be witnessed.
