(A) The number of the Torrens Title for the land on which the retirement village is located must be entered.
(B) The details of the person or firm lodging the Request with NSW Land Registry Services must be entered. If that person is not the holder of a document collection box at NSW LRS, leave the relevant panel blank.
(C) The applicant must be the operator. The full name must be stated. If the operator is a corporation, add the ABN, ACN or ARBN.
Note Where the applicant/operator is not the registered proprietor of the land a statutory declaration must be furnished:
- stating that the applicant is the operator, and
- detailing the circumstances of the applicant's appointment as operator.
(D) Place a cross in the appropriate check box.
If the second item is crossed, enter a description of the part of the land. In that case, a fully-dimensioned plan showing a pictorial representation of the part must be annexed to the dealing.
An extra fee is payable where the dealing is reliant on a plan annexed.
In the appropriate location, enter the full, official name of the retirement village, e.g. 'Southern Star Retirement Village Limited.'
(E) The Request must be executed by the applicant and be witnessed, or be executed on the applicant's behalf as follows:
